Application for the authorisation of scheduled treatment abroad
The application can be used to enforce the right to treatment abroad due to exhausted possibilities of treatment in Slovenia, excessive waiting time in Slovenia or for hospital treatment or rather treatment entailing the use of expensive medical equipment in some other EU Member State. If you have already undergone treatment, please select the application for the reimbursement of costs.
Please enter the data and click “Continue”. Obligatory data is marked with an asterisk "*". Additional explanations or instructions for data entry are available for the data marked “i”.
Files containing supporting documents must be in PDF format. The maximum permitted size of an attachment is 3MB. By clicking “Select file”, you can attach an attachment. It is possible to attach several attachments. In case of an error, you can delete an attachment by clicking “X” next to the file. To convert images to PDF format, you can use one of the applications provided for that purpose. The text in supporting documents should not be coloured with text markers.
You can send the application by clicking “Submit”. You will be informed of successful submission of your application with an email, provided you indicate your email address in the application.
The data set referring to the type of application shows the attachments that need to be enclosed to the application for the selected basis for the enforcement of right. Obligatory attachments are marked with an asterisk "*". If you enclose irrelevant attachments, you will be notified and asked to supplement them, so that we can continue the procedure.
